Client Waiver

I, _______________________, hereby acknowledge that I am voluntarily receiving massage therapy services from Galore Service at the location disclosed + agreed upon when I officially booked my appointment, which was confirmed by myself + the owner.

I understand that massage therapy involves certain risks + that it is my responsibility fully to communicate any discomfort or concerns to the therapist before, during, or after the service.

I understand that the massage therapist is not a medical provider, + that massage therapy is not a substitute for medical care. 

I acknowledge that it is my responsibility to consult with a physician regarding any medical conditions or concerns before receiving massage therapy.

I understand that the massage therapist will use professional draping techniques to ensure my privacy + comfort during the massage session. 

I agree to communicate any discomfort or inappropriate behavior from the therapist immediately.

I understand that the massage therapist may ask questions about my medical history + current health condition(s) in order to provide safe + effective treatment. I agree to provide accurate + complete information to the best of my knowledge.

I acknowledge that I have been informed of the potential benefits + risks of massage therapy, including but not limited to soreness, bruising, + allergic reactions to oils or lotions.

I understand that I have the right to refuse any massage techniques or to end the session at any time for any reason.

I release Galore Service LLC + any associates from any liability for injuries or damages that may occur as a result of the massage therapy session, except those resulting from gross negligence or intentional misconduct.

I have read + understand the terms of this waiver, + I voluntarily agree to its contents.

I, ___________________________, hereby acknowledge that I am voluntarily seeking a massage session from Galore Service for therapeutic purposes. I understand that while massage therapy can provide relaxation + therapeutic benefits, it is important to maintain appropriate behavior throughout the session. By signing this waiver, I agree to abide by the following guidelines:

1. Respectful Conduct:

I will behave respectfully towards the massage therapist at all times during the session.

2. Communication:

I will communicate openly + honestly with the massage therapist regarding any discomfort, preferences, or concerns during the session.

3. Modesty: 

I understand that I will be appropriately draped during the massage session to ensure modesty + comfort. I also understand that the removal of any or all attire is my choice completely + is no invite to act inappropriately. 

4. Professional Boundaries:

I understand that the massage therapist is a professional healthcare provider, + I will refrain from making any inappropriate or suggestive comments or gestures prior to, during or after the appointment.

5. Personal Hygiene:

I will maintain good personal hygiene by being clean + odor-free prior to the session.

6. Health Disclosure: 

I will inform the massage therapist of any medical conditions, injuries, or allergies that may affect the massage session.

7. Respect for Space: 

I will respect the massage therapist's personal space + refrain from touching them inappropriately.

8. Safe Environment: 

I understand that any behavior deemed inappropriate or unsafe by the massage therapist may result in the termination of the session immediately, + I may be asked to leave without refund.

I acknowledge that failure to adhere to these guidelines may result in the termination of the massage session, + I may be held legally responsible for any damages incurred. By signing below, I agree to abide by the terms outlined in this waiver.
